i agree to make an annual commitment. I agree to the $20 membership fee, which is payable to Holly Maier. I commit to donating $200 yearly - $100 at each of two meetings directly paid to a Charlotte County non-profit charity. I understand that I will write my $100 check to the charity selected by majority vote. If I am unable to attend, I will mail my check within seven days to the address below.
